Advance Directives

When a patient cannot make health care decisions on his/her own behalf, legally binding written instructions - called Advance Directives - may be used instead. Examples of Advance Directives include living wills, designated health care surrogates and durable powers of attorney.

If you have an advance directive, it is important that you tell the physician who admitted you. If you have any questions about advance directives, contact your lawyer, physician or clergy. You may also ask to speak with one of our social workers or chaplains.

You may print out and complete the Advance Care Plan document to the right if you do not already have one.
